What is end of life care?
End of life care supports people in the last stages of their life and their families by meeting a wide variety of needs throughout the process. This type of care is available in a number of different environments. The St Monica Trust offers end of life care in a care home setting.
End of life care at St Monica Trust
We’re nationally recognised for our award-winning palliative and end of life care, which sensitively encompasses all aspects of your physical, spiritual and emotional well-being. Available across all four of our care homes in Bristol and North Somerset, our end of life care focusses on ensuring that pain is well-managed and that your personal wishes are respected at all times.

Yes - we’re able to offer end of life care at all of our care homes in Bristol and North Somerset.
Continuing Health Care (CHC) Fast Track funding is a type of funding given by the NHS to someone who is considered to be approaching end of life. You can find more information here: https://www.nhs.uk/conditions/social-care-and-support-guide/money-work-and-benefits/nhs-continuing-healthcare/
Typically people are referred to us for end of life care from the Continuing Health Care team, who will call to see if we have availability for someone to move in with us. From there they will arrange admission with us.
